Option Care Health in Staten Island, NY is seeking a Clinical Transition Specialist (RN) to educate patients and families about home infusion services and coordinate care transitions from hospital to home or ATS. You will collaborate with discharge planners, case managers, and internal teams to ensure timely starts and high-quality patient care.

The ideal candidate is an RN with at least 2 years in healthcare, strong relationship-building, and proficient in MS Office.

Qualifications

• Licensed RN in state of practice with at least 2 years in healthcare.
• Experience building relationships with organizations and vendors.
• Knowledge of ACHC/URAC standards.
• Proficient in MS Office; internet/email.
• Will travel to care facilities and hospitals.

Responsabilités

• Proactively grows referrals and educates staff on services.
• Coordinate care transitions to home or alternate sites.
• Educate patients and families about options and services.
• Collaborate with intake, case managers, discharge planners.
• Maintain patient confidentiality and HIPAA compliance.
