Job summary

Option Care Health, Inc. is seeking a Clinical Transition Specialist in Hayward, CA. The role sells home infusion services to discharging patients and educates patients, families, and facility staff on our services and products.

You will coordinate with internal intake and external case managers to ensure quality and timely transitions. As a healthcare sales professional, you will build relationships with referral sources and work with Account Managers to grow referrals and achieve sales goals,

Qualifications

• Licensed RN or Pharmacist in the state of practice and at least 2 years in healthcare.
• Experience providing customer service to internal and external customers and meeting quality standards.
• Experience establishing relationships with individuals at all levels of organizations.
• Basic PC skills including internet, email, Word, Excel, and PowerPoint.

Responsibilities

• Proactively maintains and grows relationships with referrals sources to increase sales and patient starts.
• Interacts with area service providers to sell services and assess patient suitability for home/alternate site care.
• Educates patients, families, and facility staff about services and products.
• Provides hands-on in-person education to patients and caregivers for timely discharge.
• Communicates with staff and patients to implement effective treatment plans.
• Collects and verifies pre-admission information and signs off on documents.
• Partners with Account Manager to execute area growth plans.
• Delivers education programs to patients, caregivers, and referral sources.
• Reviews medical records to obtain history and insurance information.
• Initiates care transition coordination with referral sources and internal partners.
