Job summary

Northern Light Home Care and Hospice in Waterville (hybrid option) seeks an RN Clinical Team Coordinator to act as the central point of contact for patients and clinical staff, coordinating initial care assignments, scheduling, and patient concerns while supporting the supply management process. The role requires at least 2 years of relevant experience, BLS certification, and strong communication.

Valid driver’s license and travel within regions may be required.

Qualifications

• 2+ years of relative work experience required.
• BLS for Healthcare Providers certification is required.
• Ability to multi-task in a fast-paced environment.

• Proficient in using electronic health records (EHR) and related systems.

Responsibilities

• Triages phone calls and processes physician orders as appropriate for the level of current licensure/certification.
• Uses EHR and electronic resources to communicate with internal and external customers.
• Assigns patient visits and updates staff schedules in the EHR to support patient care needs.
• Obtains referrals and assigns clinical team members based on geography and availability.
• Collaborates with leadership to prioritize unassigned visits and adjust workloads.
• Generates and distributes team reports as directed.
