Qualifications

• Associate's degree in Nursing is required.
• Current, unrestricted RN license in the state of residence.
• 3+ years of hospital/acute care experience.
• Proficiency with Windows-based systems.

Responsibilities

• Coordinate patient care processes across the continuum.
• Develop and maintain discharge plans with patients and families.
• Collaborate with multidisciplinary teams to optimize LOS and care transitions.
• Communicate plan of care in medical records and with care teams.
• Support utilization management and post-acute services.
