Qualifications

• Associate's degree in Nursing is required.
• Proficient typing and navigation of Windows-based systems.

Responsibilities

• Coordinate patient care processes to achieve quality outcomes and optimize resource use.
• Collaborate with patients, families, and the multidisciplinary team to plan post-discharge care.
