Job summary

Alternate Solutions Health Network seeks an RN Clinical Field Staff Liaison to support field clinicians, monitor compliance, and ensure safe delivery of home health services. You will work across field and office settings, traveling as needed and collaborating with physicians, patients and families to maintain high standards of care.

The role requires active RN licensure in the employing state, three years of acute care RN experience, and preferred home health experience.

Qualifications

• Active RN license in the state of employment
• Compact licensure required within 30 days for participating states
• 3 years of RN experience in acute care setting

Responsibilities

• Serve as a resource for field staff, referral sources, physicians, patients and families on clinical issues
• Facilitate calls to and from Field staff; use resources to resolve issues and report concerns to manager
• Work in the field with staff and patients as needed
• Maintain positive relationships with referral sources including physicians and hospitals
• Assist with monthly Case Manager, Nurse and Therapy meetings
• Ensure Start of Care compliance
• Ensure quality and safe delivery of home health services
• Monitor Home Health Aide supervisory visits
• Focus on Outcome Monitors as assigned
• Implement quality improvement programs in the field
• Complete field visits as needed
• Participate in cost management of visits per episode
• Review CFSS workflow and relevant reports
• Review clinical documentation per agency protocol and Medicare guidelines
• Maintain active RN license in state of employment
• Participate in on-call, evening/weekend shifts and patient care as needed
• Participate in in-service program development
• Maintain professional appearance as company representative
• Follow agency policies and procedures
• Other duties as assigned
