Responsibilities

• Provide direct patient care in home health and hospice settings as needed.
• Assist with oversight of daily clinical operations and workflow coordination.
• Support clinical staff through mentoring, education, and leadership guidance.
• Collaborate with interdisciplinary teams to ensure high-quality patient outcomes.
• Assist in maintaining compliance with state, federal, and accreditation standards.
• Participate in care planning, case reviews, and quality improvement initiatives.
• Monitor clinical documentation for accuracy and regulatory compliance.
• Support staff scheduling, patient assignments, and operational needs.
• Help implement policies and procedures that promote patient-centered care.
• Serve as a clinical resource for nursing and support staff.
