Job summary

Northern Light Health, in partnership with Optum, seeks a Care Manager, RN to lead patient-centered care coordination across the continuum and develop safe discharge plans. You will collaborate with patients, families, and a multidisciplinary team to optimize transitions in care, LOS, and reimbursement within a telecommuting framework.

Qualified candidates have an associate degree in nursing, current RN licensure, and 3+ years in hospital or direct care settings.

Qualifications

• Associate's degree in Nursing (RN) is required.
• Current, unrestricted compact RN license in the state of residence.
• 3+ years of experience in a hospital, acute care, or direct care setting.
• Proficiency typing and navigating a Windows-based environment.

Responsibilities

• Coordinate patient care processes to achieve quality outcomes.
