Qualifications

• Graduate from an accredited School of Nursing with RN licensure.
• BSN preferred and BLS required per policy.
• Minimum one year of related field experience.

Responsibilities

• Provides clinical administrative support and liaison with physician practices and hospital case management.
• Analyzes data to prioritize multi-issue situations for timely actions.
• Identifies barriers and coordinates with leadership to resolve issues promptly.
• Reviews and supports patient care plans and communicates with involved parties.
• Supports clinical staff in care delivery and coordination of services.
• Evaluates progress through documentation reviews and team meetings.
• Supports billing compliance and timely CSC workflow per payer rules.
• Delegates care elements per policy and collaborates with patients and families.
• Utilizes resources to plan safe, effective, and financially responsible care.
• Participates in committees and facility governance activities.
