Job summary

Advocate Health is seeking a PRN Home Health Registered Nurse in Monroe, NC to manage a patient caseload, coordinate a multidisciplinary team, and ensure timely, quality care per physician orders.

The role requires an RN license (state-specific) and BLS certification, with BSN preferred. You will assess patients, document in EMR, and focus on reducing readmissions and optimizing outcomes in the home setting.

Qualifications

• Graduate from an accredited School of Nursing.
• RN license in the state you work and reside.
• BLS certification required per policy guidelines.

Responsibilities

• Manage an assigned patient caseload in home health, ensuring timely care and documentation per plan of care and MD orders.
• Collect data from patient assessments, telephonic contacts, and EMR for analysis.
• Identify high-risk patients and coordinate with the multidisciplinary team for appropriate care.
• Prioritize issues affecting patient safety and timely service delivery.
• Communicate care plan with involved parties and supervise LPNs/HHAs.
• Perform ordered treatments and evaluate progress toward patient goals.
