Job summary

Atrium Health Hospice Union in Monroe, NC is seeking an experienced RN for a full-time home health role with on-call responsibilities. Qualifications

• Graduate from an accredited School of Nursing; BSN preferred; BLS required.
• Current RN license in the state where you work; temporary license acceptable if applicable.
• Additional education, training, certifications, or experience may be required by the nurse leader.

Responsibilities

• Manage a daily patient caseload with timely care and documentation per plan of care and MD orders.
• Collect data from home assessments, telephonic contacts and EMR for care decisions.
• Analyze data to identify high-risk patients and coordinate case conferencing to mitigate risk.
• Supervise and coordinate a multidisciplinary clinical team including LPNs and HHAs.
• Perform treatments and procedures in accordance with Nurse Practice Act and agency policies.
