Job summary

Atrium Health Hospice Union in Monroe, NC is seeking a Registered Nurse to manage a patient caseload in the home setting. You will oversee a multidisciplinary team, document care per plan, and ensure high-quality outcomes under physician orders.

Qualifications

• Graduate from an accredited School of Nursing.
• BLS required per policy guidelines.
• Current RN license or temporary license as a Registered Nurse Practitioner in the state you work and reside.
• Additional education, training, certifications, or experience may be required within the department by the nurse leader.

Responsibilities

• Manage an assigned patient caseload and ensure care and documentation meet plan of care and MD orders.
• Collect data from home visits, telephonic contacts, and EMR.
• Analyze data to identify issues and high-risk patients, coordinating interventions.
• Prioritize issues to ensure patient safety and timely services.
• Identify barriers (language, literacy, etc.) and escalate appropriately.
• Develop and oversee a plan of care communicated to involved parties.
• Direct care coordination with a multidisciplinary team and supervise LPNs/HHAs.
• Perform prescribed treatments and evaluate progress toward goals.
