Job summary

Advocate Health’s Hospice Administration in Monroe, NC seeks a full-time clinical admin/ RN role providing liaison between physician practices and hospital case management. The position requires nursing licensure in the state, BLS, and at least one year of related experience, with BSN preferred.

Hours are 40 per week, with Monday–Friday schedule and rotation weekends/holidays; it is an on-site role in the Monroe region, focusing on safe, high-quality patient care and effective care coordination.

Qualifications

• RN license required in state of work.
• BSN preferred.
• BLS certification required per policy.
• Minimum 1 year experience in a related field.

Responsibilities

• Provides clinical administrative support and liaison with physician practices and case management.
• Analyzes data to prioritize multi-issue situations affecting patient care and safety.
• Identifies barriers and follows up with leadership to address resolutions.
• Reviews and supports development of patient-specific care plans and communicates them to involved parties.
• Supports clinical staff in plan implementation and coordination of care delivery.
• Evaluates progress through documentation review and team meetings.
• Supports billing compliance and timely CSC workflow completion, ensuring reimbursement.
• Delegates care tasks per legal/policy parameters.
• Collaborates with patients, families, and others in clinical practice.
• Uses resources to plan and provide safe, effective, and financially responsible clinical services.
• Participates in committees and governance activities.
