Job summary

ProHealth Physicians, part of the Optum family of businesses, is seeking a Clinical RN Lead to oversee daily clinical operations at the Vernon, CT practice. You will triage patients, coordinate care, and lead a team to deliver patient-centered services within a comprehensive care setting.

Qualifications

• Active unrestricted RN licensure in CT.
• 1+ years of nursing experience in a hospital or medical office.
• Experience with charge/clinical lead responsibilities.
• Intermediate level of computer proficiency in Microsoft Office.

Responsibilities

• Triage patients and manage phone/ message routing using EHR.
• Outreach to patients for appointments and follow-ups.
• Educate patients on illness/injury and self-management.
• Document patient interactions accurately in EHR.
• Carry out standing orders and ensure timely responses to tasks.
