Job summary

BLEHEALTH is seeking an RN Clinical Consultant in Pomona, CA to lead clinical oversight within the Enhanced Care Management (ECM) program and connect high-need members with medical, behavioral health, housing, and community resources. This is a full-time, in-office role supporting a mission-driven, interdisciplinary team.

Qualifications

• Active California RN license.
• 3+ years healthcare management or 4+ years clinical nursing.
• Experience in Care Management, Population Health, ECM or CalAIM preferred.
• Excellent communication, clinical decision-making, and care coordination.
• Valid California driver’s license and reliable transportation.

Responsibilities

• Provide clinical oversight and guidance to Care Managers, Housing Navigators, and interdisciplinary care teams.
• Support members with complex medical, behavioral health, and social needs through ECM.
• Collaborate with PCPs, Behavioral Health, hospitals, specialists, and managed care plans.
• Review and approve assessments, care plans, and updates.
• Assist with care transitions, discharge planning, medication reconciliation, and readmission prevention.
• Provide clinical consultation for high-risk and complex cases.
• Participate in case conferences, chart reviews, and quality improvement initiatives.
• Support outreach including home visits, hospital visits, shelters, and community settings as needed.
• Monitor documentation, outcomes, and ECM compliance.
• Train, mentor, and support staff to ensure high-quality, member-centered care.
