Job summary

Pacific Health Group is seeking an RN with clinical leadership to serve as the Clinical Case Consultant within the CalAIM ECM program. The role combines clinical guidance, field visits, and supervision of LVNs to ensure high‑quality, member‑centered care across high‑risk Medi‑Cal populations.

The position is hybrid in California, involves substantial travel, and emphasizes collaboration with care teams, hospitals, plans, and community partners to improve outcomes and reduce avoidable utilization.

Qualifications

• Active California RN license required.
• Minimum 2 years direct clinical experience in community health, managed care, or related settings.
• Experience with Medi-Cal populations and complex needs preferred.
• Ability to travel up to 60–70% within the county; hybrid/field-based role.
• Strong communication, supervision, and documentation skills.

Responsibilities

• Serve as the primary clinical resource for ECM care teams.
• Provide clinical guidance to care managers, LVNs, CHWs, and team members.
• Monitor high‑risk members and guide care planning and risk mitigation.
• Conduct field visits, shadow team members, and provide real‑time coaching.
• Supervise LVNs and ensure compliance with regulatory standards.
• Collaborate with providers, hospitals, and community partners to coordinate care.
• Develop and deliver clinical training and support staff development.
• Support quality improvement and program development initiatives.
