Job summary

BLEHEALTH is seeking a Registered Nurse (RN) Clinical Consultant to lead Enhanced Care Management (ECM) across care teams in Pomona, CA. You will oversee care managers, housing navigators, and interdisciplinary teams to improve outcomes for high-need members.

The role requires CA RN licensure and 3+ years in healthcare management or 4+ years clinical nursing in related fields. This full-time, in-office position offers collaboration with care teams and community partners.

Qualifications

• Active California Registered Nurse license.
• 3+ years of healthcare management experience or 4+ years of clinical nursing experience in home health, primary care, managed care, care management, or community health.
• Prior supervisory, leadership, or administrative experience preferred.
• Experience in Care Management, Case Management, Population Health, Behavioral Health, Managed Care, ECM, or CalAIM strongly preferred.
• Excellent communication, clinical decision-making, and care coordination skills.
• Valid California driver's license and reliable transportation.

Responsibilities

• Provide clinical oversight and guidance to Care Managers, Housing Navigators, and interdisciplinary care teams.
• Support members with complex medical, behavioral health, and social needs through the Enhanced Care Management (ECM) model.
• Collaborate with Primary Care Providers, Behavioral Health providers, hospitals, specialists, and Managed Care Plans.
• Review and approve assessments, care plans, and care plan updates.
• Assist with care transitions, hospital discharge planning, medication reconciliation, and readmission prevention.
• Provide clinical consultation for high-risk and complex cases.
• Participate in case conferences, chart reviews, and quality improvement initiatives.
• Support outreach efforts, including home visits, hospital visits, shelters, and community-based settings as needed.
• Monitor documentation, outcomes, and compliance with ECM and Managed Care requirements.
• Train, mentor, and support staff to ensure high-quality, member-centered care.
