Job summary

WellSense Health Plan seeks a Clinical Care Manager to deliver holistic care management for diverse member populations across the continuum of care. You will perform assessments, develop care plans, coordinate services, and promote self-management through motivational interviewing and evidence-based practices.

You will work with families, providers, and community resources to address social determinants of health, ensure appropriate utilization, and support transitions of care through

Qualifications

• Bachelor's degree in Nursing required; RN licensure in the state of employment.
• Master’s or doctoral degree in a behavioral health field preferred.
• Minimum three years of clinical experience with chronic/complex conditions.

Responsibilities

• Completes comprehensive assessments, reconciliations, and evaluations to identify health and social needs.
• Collaboratively develop, implement, and monitor individualized care plans.
• Coordinate services across primary, specialty, behavioral health, and community resources.
• Promote self-management through motivational interviewing and education.
• Support transitions of care and ensure appropriate utilization of services.
