Job summary

WellSense Health Plan in Boston-area Massachusetts is seeking a Clinical Care Manager to provide holistic care management for members. The role involves assessments, care planning, education, coordination across care teams, and ongoing monitoring to improve health outcomes and self-management.

You will engage diverse member populations, work with multisector partners, and use telephonic and in-person approaches to promote adherence and optimal utilization of services.

Qualifications

• Bachelor's degree in Nursing is required
• Master's or doctoral degree in a behavioral health field is preferred
• Associate Degree in Nursing with relevant experience may be considered
• Must hold a current state RN license and be eligible to practice in MA/NH

Responsibilities

• Complete comprehensive assessments and develop individualized care plans.
• Coordinate services across primary care, behavioral health, and community resources.
• Educate members and caregivers on disease processes and self-management strategies.
• Promote transitions of care and post-discharge follow-up.
• Utilize motivational interviewing and evidence-based engagement techniques.
• Document assessments and interventions in care management systems.
