WellSense Health Plan is seeking a Clinical Care Manager in the Boston area. This role delivers holistic clinical care management across the continuum, including assessments, care planning, education, and coordination with families, providers, and community resources.

The incumbent will work with diverse member populations, including pregnant and postpartum individuals, infants, and patients with chronic and complex conditions, using telephonic outreach, virtual engagement, and face-to-face

Qualifications

• RN with state licensure required.
• Minimum 3 years of clinical experience with high-risk patients.
• Experience in Medicaid populations and care management preferred.

Responsabilités

• Conduct comprehensive assessments and care planning.
• Coordinate services across primary, specialty, behavioral health, and social supports.
• Engage members through motivational interviewing and education.
• Document interventions and outcomes in EMR and care management systems.
