Job summary

WellSense Health Plan is seeking a Clinical Care Manager to deliver holistic care management for diverse members across the continuum of care, including assessment, care planning, education, and coordination.

This role emphasizes whole-person care, collaboration with families, providers, and community partners, using motivational interviewing and evidence-based practices to improve outcomes and reduce barriers to care.

Qualifications

• RN license required (Massachusetts) or state of employment.
• Minimum three years of clinical experience with chronic/high-risk health conditions.
• Experience with Medicaid populations and population health is preferred.

Responsibilities

• Perform comprehensive assessments, medication reconciliations, and condition-specific evaluations to identify needs.
• Collaboratively develop, implement, monitor, and update individualized care plans.
• Educate members and caregivers on disease processes, preventive care, and self-management.
• Coordinate services across primary, specialty, behavioral health, pharmacy, and community resources.
• Promote transitions of care and address barriers such as transportation and housing.
• Document interactions and outcomes in care management systems.
