Job summary

Jobtailor is seeking a California-licensed Registered Nurse to coordinate clinical care for ALWP clients in the Torrance area. The role requires developing ISPs, assessing client needs, and ensuring timely service delivery while maintaining comprehensive documentation and regulatory compliance.

Responsibilities include telehealth visits, risk reporting, case conferences, and continuous quality improvement activities.

Qualifications

  • Must hold a current, un-revoked California RN license.
  • Knowledge of California Assisted Living Waiver Provider Handbook and RCFE Provider Handbook.
  • Knowledge of CCT, ALWP transition process, and Nursing Practice Act.

Responsibilities

  • Coordinate clinical care for ALWP clients under the Care Coordination Agency.
  • Identify, organize, coordinate, and monitor services needed by recipients.
  • Assist waiver recipients in accessing waiver services, state plan services, and community resources.
  • Act as the clinical resource for the CCT Program.
  • Complete clinical documents required for the State Waiver Program Application.
  • Conduct assessments using the ALW Assessment Tool and determine each client’s level of care.
  • Develop and modify Individualized Service Plans (ISPs).
  • Monitor service delivery according to the prescribed client-contact schedule.
  • Conduct monthly and as-needed face-to-face or supervisor-approved telehealth visits.
  • Maintain charting, progress notes, and case records for enrolled clients.
  • Manage complaints from clients, families, or friends.
  • Report abuse or neglect to the appropriate agencies.
  • Attend monthly or as-needed Case Conferences with RCFE facilities.
  • Evaluate ISP effectiveness through client observation and communication with clients and health team members.
  • Review Significant Incident Reports with clients and RCFE staff as necessary.
  • Provide clinical support to CCA Office Coordinators.
  • Participate in ALWP education, training, and quality improvement activities.
  • Assist with compliance with Federal, State, Local, HIPAA, and Joint Commission requirements.
  • Establish and maintain relationships with RCFE, SNFs, and other vendors.
  • Self-assess nursing skills and educational needs.
  • Drive to geographical areas covered by the company as required.
  • Maintain proper timekeeping, follow Employee Handbook policies, report fraud and abuse, conduct mandated reporting, document client contact, attend department meetings, and perform other assigned duties.

Requirements
  • Must have a current, unsuspended, un-revoked license to practice as a Registered Nurse in California
  • Knowledge of The State of California Assisted Living Waiver Care Coordination Agency Provider Handbook and RCFE Provider Handbook
  • Knowledge of the clinical requirement of the CCT and ALWP transition process
  • Knowledge of the current Nursing Practice Act
  • Current CPR certification
  • Proficient in the use of computers
  • Minimum two (2) years’ experience in a home health organization, primary care clinic, or a health facility
  • One (1) year experience in healthcare quality management, preferred
  • Current and satisfactory pre-employment physical examination including TB Screening Test or chest X-ray as required by Agency policies and procedures
  • Must be free from signs of infection and illness
  • Demonstrated exemplary clinical, analytical, and documentation skills
  • Good written and verbal communication skills
  • Fluent in English
  • Must be a licensed driver with an insured automobile in good working order, in accordance with state and/or organizational requirements
  • Ability to meet physical requirements, including frequent computer/telephone/keyboard use, bending, walking, driving, and lifting/pushing up to 25 lbs
  • Familiarity with Title 22
  • Must maintain all required credentials up-to-date
  • Must attend all state mandated in-service trainings
