Qualifications

  • Excellent interpersonal, communication, and engagement skills.
  • Proficiency in electronic health records and basic computer systems.
  • Must be proficient in technology, including documentation systems, EHR platforms, and communication tools.
  • Ability to document in real-time and manage multiple systems effectively.
  • Ability to travel extensively within assigned county.
  • Must reside within assigned service area.
  • Ability to work occasional evenings or weekends for community events.
  • Ability to work in a hybrid, field-based environment.
  • Possess a valid California Driver’s License (Class C minimum)
  • Maintain a personal, operable vehicle for daily business use
  • Maintain current liability insurance meeting California’s minimum legal requirements
  • Must pass a Motor Vehicle Report (MVR) background check prior to employment

Responsibilities

  • Conduct comprehensive assessments and develop Plans of Care
  • Provide ongoing member support and follow-up
  • Assist with care navigation, appointments, service coordination, and transitions of care
  • Deliver health education and coaching
  • Address transportation, language, and access-to-care barriers
  • Conduct outreach to high-risk and hard-to-reach populations
  • Participate in community events, wellness fairs, mobile health clinics, vaccination drives, and outreach initiatives
  • Facilitate group sessions and workshops on health, prevention, mental health, substance use, and wellness
  • Assist with public assistance enrollment and advocate for access to care and services
  • Maintain timely documentation and complete reporting and billing documentation
  • Collaborate with interdisciplinary teams including RNs and licensed providers
  • Participate in case conferences and team meetings
  • Travel within the assigned county and work primarily in field-based settings
