Job summary

Jobtailor in the United States is seeking a dedicated registered nurse to deliver patient care in home health settings. You will follow assessment, planning, implementation, and evaluation to ensure high-quality nursing services.

Qualifications

  • Graduation from an approved program reflecting ability to perform the job as described
  • Holds a valid/unencumbered nursing license to practice in the state of service
  • Have professional experience as required per state regulations to perform the job as described
  • Successful completion of competency testing with a score of 80% or greater
  • Reliable transportation
  • Provide copy of current TB results or Chest X-ray
  • Provide copy of CPR certification per agency policy
  • Be flexible, organized and exercise independent judgment
  • Regularly sit, stand, bend, kneel, walk, push/pull and reach as required by duties
  • Ability to travel frequently for home health visits

Responsibilities

  • Deliver patient care to assigned patients using assessment, planning, implementation, and evaluation
  • Provide skilled nursing according to the plan of treatment and nursing care plan
  • Perform skilled nursing visits in accordance with physician orders
  • Identify and follow through on new problems, changes, and resolutions
  • Notify the Director/Supervisor and physician of patient changes and obtain necessary intervention orders
  • Document clinical notes, supplemental physician orders, and nursing care plans
  • Observe aseptic technique as required
  • Perform specific treatments and medication administration according to the care plan
  • Function as a team member under the direction of the registered nurse
  • Communicate skilled observations and significant patient-status changes to the Director of Clinical Services and/or Clinical Manager
  • Request modified or additional orders based on clinical status and nursing judgment
  • Document physician collaboration in the medical record
  • Utilize and document appropriate available services
  • Participate in mandated agency activities such as in-services
  • Complete clinical documentation accurately and timely
  • Represent the company professionally and adhere to dress code
  • Report on time and as scheduled
  • Provide home health visits in private homes and travel frequently

Job description

Requirements


  • Graduation from an approved certification/program/accredited school/undergraduate/graduate school or post graduate education to reflect ability to perform the job as described

  • Holds a valid/unencumbered verification/license to practice the job as described in the state where services are provided

  • Have professional experience as required per state regulations to perform the job as described

  • Successful completion of assigned screening/competency testing with a proficiency score of 80% or greater

  • Reliable transportation

  • Provide copy of current TB results or Chest x-ray

  • Provide copy of current CPR certification as per agency policy

  • Be flexible, organized and have the ability to exercise independent and sound judgment to perform the job duties/responsibilities

  • Ability to regularly sit, stand, stop, bend, kneel, walk, balance, crouch, crawl, push, pull and reach with hands and arms and other physical activities as deemed necessary by job duties

  • Vision and hearing capabilities within normal limits, with or without aid

  • Ability to frequently lift and/or move up to 15 pounds and occasionally lift or move 100 pounds or more if applicable

  • Ability to perform the anticipated level of physical assist required to provide safe client care

  • Ability to travel frequently to complete home health visits
