Job summary

myPlace Health seeks an LVN to coordinate and deliver in-home nursing care across Los Angeles. You will perform assessments, plan care, and work with an interdisciplinary team to ensure high-quality service for frail adults.

The role requires travel 50% between Downtown LA and Compton locations, active CA LVN license, CPR/First Aid within 60 days, and strong communication in a multidisciplinary setting. Emphasis on safety and compassionate patient care.

Qualifications

Responsibilities

• Coordinate home care services via contracted agencies per care plans.
• Participate in interdisciplinary team through assessments and planning.
• Provide nursing care within LVN scope in homes and clinics.
• Monitor conditions and report changes to clinical team.
• Educate participants and caregivers on in-home care and safety.
• Address participant questions and coordinate logistics for visits.
• Oversee agencies for quality, compliance and consistency.
• Document care and activities in the electronic medical record.
• Assist with care plan updates and staff orientation.
• Coordinate durable medical equipment and related needs.

Job description

• Coordinate home care services through contracted agencies according to individualized plans of care, authorized needs, and program standards


• Participate in the Interdisciplinary Team through home assessments, reassessments, change-of-condition evaluations, care planning, and ongoing review of home care needs


• Provide nursing care within the LVN scope of practice, including wellness checks, medication administration, wound care, and other clinical services in homes, clinics, and day centers


• Monitor participant condition and service delivery, reporting changes, treatment concerns, and adverse reactions to appropriate clinical team members


• Educate participants and caregivers on in-home care, safety, and self-management


• Respond to participant and family questions or concerns about home care services


• Schedule home care visits and coordinate appointment logistics and transportation


• Oversee contracted home care agencies for quality, consistency, and compliance


• Document nursing care, coordination, assessments, and service activities in the electronic medical record


• Participate in care coordination meetings, update care plans, and assist with training and orientation of new home care staff


• Coordinate durable medical equipment and related needs


• Support myPlace Health's vision and goals and perform other duties as assigned

Requirements

• Current, active Licensed Vocational Nurse (LVN) license in the State of California


• CPR and First Aid certification required within 60 days of hire


• Experience providing care to frail and elderly adults


• Travel 50% of the time covering both Downtown Los Angeles & Compton locations


• Valid California driver's license, insurance and reliable personal transportation


• Strong verbal, written, and organizational skills


• Ability to work effectively in a multidisciplinary team environment


• Proficiency in computer applications, typing, and electronic medical record systems


• Ability to maintain professionalism, exercise sound judgment, and respond effectively under pressure


• Demonstrated dependability, flexibility, and commitment to quality and ethical standards


• Compassion, patience, and respect in working with older adults and their families


• Prior experience in home care or community-based care settings preferred


• Proof of Tuberculosis screening upon hire or proof of a negative screening within the last year, subject to the stated accommodation process
