Job summary

Jobtailor is hiring an RN/LSW/LCSW/LMHC licensed care coordinator to assess members at their homes and coordinate personalized care plans in Indiana. You will work with an interdisciplinary team to address chronic illnesses and behavioral health needs, ensuring appropriate care levels and supports.

Travel to member locations is required, with a Monday–Friday, 8am–5pm schedule. Applicants should have strong communication and interpersonal skills, and a compassionate, patient-centered approach to

Qualifications

• Current, unrestricted RN or licensed social work/mental health professional credentials as applicable.
• Experience with chronic illnesses, co-morbidities, and disabilities in a care coordination role.
• Ability to travel to members' homes and other locations as needed.

Responsibilities

• Conduct assessments to identify members' health and support needs.
• Identify high-risk members and those who may need alternative levels of care.
• Obtain clinical data as directed by RN or care coordinator.
• Coordinate care for members with chronic illnesses and disabilities.
• Collaborate with interdisciplinary team to implement service plans.
• Travel to members' homes and other locations to provide support.
• Focus on behavioral health needs within clinical assessments.

Job description

Conduct assessments to identify members' physical health, behavioral health, social services, and long-term services and supports needs
Identify members at high risk for complications and those who may benefit from alternative levels of care or waiver programs
Obtain clinical data as directed by the responsible RN or care coordinator
Coordinate care for members with chronic illnesses, co-morbidities, and disabilities
Collaborate with the interdisciplinary team to develop and implement person-centered service plans
Participate in team rounds and collaborate with care coordinators to ensure effective care coordination
Travel to members' homes and other locations to conduct assessments and provide support
Focus on clinical assessments and support for members' behavioral health needs

Requirements

• Current, unrestricted RN, LSW, LCSW, or LMHC license in applicable state(s) required
• Minimum of 2 years of experience working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator or similar role, or equivalent combination of education and experience
• State-specified certification based on state law and/or contract may be required
• Strong communication and interpersonal skills
• Ability to work effectively as part of an interdisciplinary team
• Compassionate and patient-centered approach to care
• Ability to travel to members' homes and other locations as necessary
• Monday-Friday, 8am-5pm availability
