Jobtailor is seeking an experienced Registered Nurse to manage patient caseloads, conduct thorough assessments, and develop and adjust plans of care in collaboration with physicians and families. Qualifications

  • Associate's Degree in Nursing is required.
  • RN license issued by the state of practice is required.
  • BLS for Healthcare Providers through the American Heart Association must be obtained within 6 months unless exempt.
  • Valid driver license with insured vehicle.
  • Typically requires 3 years of clinical nursing experience.
  • Excellent communication and interpersonal skills.
  • Ability to work with physicians and other professionals.
  • Proficiency in clinical skills and ability to work under direction and make sound judgments.

Responsibilities

  • Manage caseloads of patients in specialty areas.
  • Perform initial assessments to gather physical, social, and environmental data.
  • Establish care plans per physician orders and patient needs.
  • Provide direct patient care and adjust nursing processes as needed.
  • Identify problems, modify care plans, and evaluate outcomes.
  • Consult with families and other healthcare personnel.
  • Monitor patient responses to treatments and teaching; apply interventions.
  • Discharge planning and ensuring continuity of care after discharge.
  • Assist in chart audits and quality-improvement activities.
  • Maintain accurate patient records per guidelines.

Job description

  • Assume responsibility for caseloads consisting of patients in specialty areas
  • Conduct initial assessments to obtain thorough physical, social, and environmental data
  • Establish plans of care according to physician orders and patient needs
  • Provide direct patient care and adjust nursing care processes as needed
  • Identify problems, modify nursing care plans, and evaluate outcomes
  • Advise and consult with families and other personnel
  • Assess patient responses to treatments and teaching and apply interventions
  • Update and revise plans of care
  • Complete visit assessments, OASIS, 485, and verbal-order documentation accurately and promptly
  • Communicate with physicians to confirm and update patient plans of care
  • Provide status updates and maintain communication with physicians and healthcare-team members
  • Facilitate discharge planning and assure continuity of care after discharge
  • Participate in developing discharge plans with patients, families, physicians, social workers, and staff
  • Participate in chart audits and continuous quality-improvement activities
  • Identify, develop, and participate in process-improvement opportunities
  • Maintain and update accurate clinical and patient records according to agency, state, and federal guidelines
  • Provide age-appropriate care based on patient assessment and departmental policies and procedures
Requirements
  • Associate's Degree in Nursing
  • Registered Nurse license issued by the state in which the team member practices
  • Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) needs to be obtained within 6 months unless department leader has determined it is not required
  • A valid driver's license issued by the Division of Motor Vehicles, or driver license issued by the state of Illinois
  • Typically requires 3 years of experience in clinical nursing
  • Excellent communication skills
  • Strong interpersonal skills and ability to work with a variety of clients with patience and respect
  • Good organizational, analytical, and problem-solving skills
  • Ability to work well with physicians and other professionals
  • Proficiency in clinical skills and ability to work under direction and make sound judgments
  • Ability to operate FAX, computer, copier, and telephone system
  • Ability to assess and interpret patient-status data
  • Tolerance for differences and appreciation of multiculturalism and diversity
  • Proof of a valid driver license and a registered, functional car with adequate insurance coverage
  • Physical ability to bend, twist, kneel, squat, reach overhead, sit, stand, walk, climb stairs, and work in confined spaces
  • Ability to safely work with people of various sizes, weights, medical conditions, and physical limitations
  • Physical stamina to attend to patient care requirements for three or more hours without complete rest
  • Accurate vision to read gauges, measuring devices, and written instructions
  • Ability to lift up to 50 lbs from floor to waist
  • Ability to lift up to 20 lbs from waist to overhead
  • Ability to lift and carry up to 40 lbs at waist height
  • Ability to push/pull with 30 lbs of force
  • Ability to perform a sliding transfer of 150 lbs with a second person present
