- Assume responsibility for caseloads consisting of patients in specialty areas
- Conduct initial assessments to obtain thorough physical, social, and environmental data
- Establish plans of care according to physician orders and patient needs
- Provide direct patient care and adjust nursing care processes as needed
- Identify problems, modify nursing care plans, and evaluate outcomes
- Advise and consult with families and other personnel
- Assess patient responses to treatments and teaching and apply interventions
- Update and revise plans of care
- Complete visit assessments, OASIS, 485, and verbal-order documentation accurately and promptly
- Communicate with physicians to confirm and update patient plans of care
- Provide status updates and maintain communication with physicians and healthcare-team members
- Facilitate discharge planning and assure continuity of care after discharge
- Participate in developing discharge plans with patients, families, physicians, social workers, and staff
- Participate in chart audits and continuous quality-improvement activities

- Maintain and update accurate clinical and patient records according to agency, state, and federal guidelines

Requirements
- Associate's Degree in Nursing
- Registered Nurse license issued by the state in which the team member practices
- Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) needs to be obtained within 6 months unless department leader has determined it is not required
- A valid driver's license issued by the Division of Motor Vehicles, or driver license issued by the state of Illinois
- Typically requires 3 years of experience in clinical nursing
- Excellent communication skills
- Strong interpersonal skills and ability to work with a variety of clients with patience and respect
- Good organizational, analytical, and problem-solving skills
- Ability to work well with physicians and other professionals
- Proficiency in clinical skills and ability to work under direction and make sound judgments

- Physical stamina to attend to patient care requirements for three or more hours without complete rest
