Job summary

myPlace Health is seeking a CDI Specialist to ensure accurate, clinically specific provider documentation across the participant population. This clinically credentialed role partners with physicians, nurse practitioners, and care teams to support compliant CMS‑HCC capture and sustainable documentation improvement within the PACE model.

You will review records, identify opportunities, and develop provider education and templates.

Qualifications

• Active, unrestricted RN or NP license in the state of California.
• Minimum of three years of clinical experience in outpatient/ambulatory care.
• Foundational knowledge of ICD-10-CM coding principles and their relation to documentation.
• Strong written and verbal communication with physicians and care teams.
• Proficiency with EHR systems, digital workflows, and documentation tools.
• Ability to work independently in a fast-paced clinical environment.
• Attention to detail, analytical thinking, and clinical judgment.
• Experience in CDI, risk adjustment, HCC coding, preferably in value-based care.
• Familiarity with Medicare Advantage, PACE, capitated care, or similar models.
• Certification preferred; willing to obtain certification within 12 months.

Responsibilities

• Conduct prospective, concurrent, and retrospective chart reviews to identify documentation gaps and unsupported diagnoses.
• Apply clinical reasoning across EMR, labs, imaging, meds, and outside records for precise documentation.
• Review longitudinal trends to reflect disease progression and patient complexity.
• Formulate compliant provider queries with objective clinical evidence.
• Collaborate with physicians and care teams to improve CMS-HCC capture.
• Participate in meetings and workflows to surface documentation opportunities.
• Prioritize daily chart reviews based on risk stratification and organizational needs.
• Track documentation patterns, query outcomes, and capture rates for audits.
• Develop provider education materials, templates, and workflow guides.
• Conduct quality reviews and address deficiencies, risks, and systemic trends.
