Job summary

myPlace Health is seeking a CDI Specialist to support accurate provider documentation across our PACE participant population in California. You will review medical records, identify documentation opportunities, and collaborate with clinicians to ensure CMS-HCC capture reflects patient complexity and care needs.

The role involves chart reviews, drafting queries, and partnering with physicians and care teams to improve documentation quality, with a focus on risk adjustment and outpatient settings.

Qualifications

• Experience in clinical documentation integrity, risk adjustment, HCC coding, or related fields.
• Strong knowledge of ICD-10-CM coding principles and how they relate to documentation.
• Ability to interpret and synthesize information from multi-source medical records.
• Excellent written and verbal communication with clinicians and care teams.
• Proficiency with electronic health record systems and related tools.

Responsibilities

• Conduct prospective, concurrent, and retrospective reviews to identify documentation gaps and HCC opportunities.
• Formulate compliant provider queries with objective clinical evidence.
• Collaborate with physicians and care teams to improve documentation quality.
• Track documentation patterns and outcomes using CDI tools and dashboards.
• Develop provider education materials and templates to support CMS guidelines.
