Job summary

Qualifications

• Licensed Registered Nurse in CT prior to hire or transfer date.
• Preferred bilingual English/Spanish.
• Minimum three years of acute-care nursing experience; case management preferred.

Responsibilities

• Develop and implement discharge planning with the health care team.
