Qualifications

Responsibilities

• Coordinate and ensure the plan of care across the continuum of care.
• Identify and optimize hospital system efficiency affecting quality/length of stay.
• Assist clinicians in documenting the appropriateness of admissions and continued stays.
• Develop and implement a discharge plan with the interdisciplinary health care team.
• Identify service, treatment, and funding options and coordinate referrals to outside agencies.
