Job summary

Yale NewHaven Health is seeking a Case Manager in Connecticut to coordinate and implement patient care plans within the multidisciplinary team. The role emphasizes evidence-based practice, efficient resource use, and collaboration to improve patient outcomes and discharge planning.

The ideal candidate holds a Baccalaureate degree in a clinically related field, an active RN license in Connecticut, and at least three years of clinical experience.

Qualifications

• Minimum of a Baccalaureate degree in clinically related field; RN required.
• Minimum of three (3) years of relevant clinical experience.
• Active RN Licensure in Connecticut.

Responsibilities

• Coordinate and ensure the implementation of the plan of care, utilizing the principles of case management.
• Establish a system for coordinating the care of a patient throughout the continuum of care, linking inpatient and outpatient care.
• Optimize the efficiency of hospital systems which impact quality and/or length of stay.
• Identify and monitor compliance with documenting variances from established parameters in the clinical pathway or treatment plan.
• Assist clinicians in documenting the appropriateness of admissions and continued stays.
• Ensure discharge planning is developed and implemented with health care teams.
• Link patient and family with institutional or community resources and advocate for scarce resources.
