Job summary

Yale New Haven Health is seeking a Case Manager to coordinate high-value patient care within an interdisciplinary team. You will manage the plan of care, connect inpatient and outpatient services, and support discharge planning to ensure efficient resource use and patient outcomes.

The role requires RN licensure in Connecticut and at least three years of relevant clinical experience, with strong communication and leadership abilities to navigate complex care pathways.

Qualifications

• Minimum of a Baccalaureate degree in clinically related field. RN required.
• Minimum of three (3) years of relevant clinical experience.
• Active RN Licensure in Connecticut.
• Recent appropriate nursing experience, theoretical knowledge of the nursing process, case management and continuity of care.
• Advanced communication and interpersonal skills with all levels of internal and external customers.
• Leadership skills in planning and managing patient care as acquired through greater than 3 years of clinical nursing experience in an acute care hospital.
• Utilization management and case management experience preferred.
• Knowledge of computer software and hardware applications and a basic knowledge of statistics.

Responsibilities

• Coordinate and ensure the plan of care is implemented by the interdisciplinary health care team.
• Optimize hospital systems affecting quality and length of stay.
• Ensure patients meet clinical needs for admission, treatment, and discharge.
• Assist clinicians in documenting appropriateness of admissions and continued stays.
• Develop and implement an appropriate discharge plan with the care team.
• Ensure discharge plan provides continuum of care with outpatient services.
• Coordinate contacts with outside agencies and referrals.
• Work with PSM and unit leadership to involve nurses in discharge planning.
• Advocate for patients and address ethical/legal issues.

Education

Bachelor's degree in clinically related field
