Job Summary
The licensed Case Coordinator - RN demonstrates expertise in a healthcare setting in the assessment and treatment of patients along the continuum of care. This position develops and implements discharge planning within an interdisciplinary healthcare team and addresses complex clinical care needs by identifying and removing barriers that prevent optimum access to needed post‑acute care. The coordinator collaborates with providers to identify discharge barriers and delays in order to optimize the most efficient use of acute care hospital days and prevent prolonged length of stay. The position also provides clinical leadership to healthcare teams and ensures daily goals are met according to the patient’s plan of care.
Job Responsibilities
- Assess patient population in collaboration with the Health Care Team, identify risk factors, and determine level of case coordination.
- Develop and implement discharge planning, monitor plans of care, coordinate consultations and referrals, and facilitate patient and family engagement.
- Collaborate daily with direct‑care providers to assess in‑hospital stays and facilitate post‑hospitalization services, placement, and transportation.
- Model quality patient/customer service, maintain confidentiality of all patient information, and promote a collaborative environment.
- Gather comprehensive information to develop interventions and treatment strategies.
- Facilitate case management to address biomedical and psychosocial needs, ensuring efficient, appropriate care.
- Advocate for patients’ needs and system change to improve access to care.
- Educate patients, families, community, and professionals on disease prevention, health maintenance, and treatment adherence.
- Maintain timely documentation of case management services reflecting pertinent assessment, interventions, and outcomes.
- Provide timely assessments for discharge planning for patients with prolonged length of stay.
- Identify discharge delays, develop countermeasures, and reduce readmissions.
- Address payer source gaps and secure needed insurance coverage or providers for post‑acute care.
- Identify opportunities to decrease length of stay and improve patient experience through Lean huddles and interdisciplinary meetings.
- Assess patient readiness for discharge based on clinical status.
- Collaborate with providers to ensure appropriate placement and documentation support present level of care.
- Perform other related duties as required.
Qualifications
Required Credential: Licensed Registered Nurse credentialed from the Connecticut Department of Public Health prior to hire date or transfer date.
Preferred Credentials
- Certified BLS Provider from the American Heart Association (AHA).
- Certified BLS Provider from the American Red Cross.
- Case Management Certification (preferred).
Education
- Completion of a formal nursing program.
- Associate’s degree hires; Nursing Baccalaureate Degree (BSN) must be obtained within 6 years of hire.
Work Experience
- Three years of acute‑care experience, with diversified clinical nursing in medical/surgical settings.
- Case management, home care, and/or managed care experience strongly considered.
- Previous experience in multiple healthcare settings (preferred).
Knowledge, Skills, and Ability Requirements
- Comprehension of medical terminology, disease processes, and use of educational resources for continuous improvement.
- Excellent communication, negotiation, and conflict resolution skills.
- Computer application proficiency preferred.
- Ability to provide expert verbal and written documentation and consultation across the continuum of care.
- Collaboration skills with multidisciplinary healthcare professionals to ensure seamless care transitions.
- Multitasking ability in a fast‑changing healthcare environment.
- Maintenance of CEUs required for RN, CCM, and/or ACM licensure.
- Preferred bilingual English/Spanish.
