Job summary

NewYork-Presbyterian Hudson Valley Hospital is seeking an experienced RN to join its Case Management team as a Care Coordinator. In this role you will help implement the Case Management Program, provide leadership and education to team members, and contribute to safe discharge planning initiatives.

You will collaborate with clinical and administrative staff, navigate Managed Care/HMO requirements, and ensure compliant discharge planning and community services.

Qualifications

• BSN required.
• Minimum two years experience in Case Management.
• Current New York State RN license required; CCM desirable.

Responsibilities

• Assist with the implementation of a Case Management Program.
• Provide leadership and education to other members of the Case Management team.
• Develop safe discharge planning strategies.
• Communicate effectively with administrative and professional staff.
• Make decisions about discharge planning, home health situations, and community services.
• Understand Managed Care/HMO requirements, laws, and regulations.
