Job summary

Key duties include assessing medical status, developing care plans, ensuring timely return to baseline, documenting activities, and complying with insurance requirements. Travel within the region may be required.

Qualifications

• Active and unrestricted CT license.
• Minimum 3+ years clinical practical experience (diabetes, CHF, CKD, post-acute care, hospice, palliative care, cardiac) with Medicare members.
• Minimum 2+ years CM, discharge planning and/or home health care coordination experience.
• Registered Nurse with active state license in good standing.
• Travel within a designated geographic area for in-person case management as needed.
• Bachelor’s degree preferred, CCN/CM certifications preferred.

Responsibilities

• Facilitate delivery of benefits and healthcare information to determine eligibility while promoting wellness activities.
• Develop and support health strategies, policies, and programs for member wellness and timely return to work.
• Act as liaison with members, employers, providers, and insurers as appropriate.
• Implement and coordinate case management for catastrophic or chronically ill members across care continuum.
