CVS Health is seeking a Community Care Case Manager in Connecticut. You will use a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy to meet health and social needs. Travel is required to support members and providers in the CT region.

The role focuses on ensuring appropriate benefits delivery, wellness, and timely return to work, through collaboration with internal teams and external partners in a patient-centered model.

Qualifications

• Active RN license required.
• Associate degree in nursing or nursing diploma required.
• BSN preferred.

Responsibilities

• Coordinate case management activities for members across the continuum of care.
• Communicate with members, providers, and insurers to facilitate care plans.
• Assess member needs and determine eligibility for benefits and services.
• Document all case management activities and outcomes.
