Qualifications

• RN licensure in the state of operation.
• Graduate of an accredited School of Nursing.
• CCM preferred but not required.

Responsibilities

• Provide medical case management coordinating with patients, physicians, employers, and referrals.
• Apply Case Management steps: assessment, planning, implementation, evaluation, outcomes.
• Evaluate treatment plans for medical necessity and cost-effectiveness.
• Coordinate delivery of equipment and home health services.
