Job summary

CVS Health is seeking a full-time Community Care Case Manager (RN) to join our patient-centric team in Hartford area, with travel up to 55-75% around Yale New Haven CT, and 40-hour weeks. The role emphasizes assessment, care coordination, and advocacy to improve outcomes.

Requires Associate's Degree in Nursing or Diploma; CT RN license; 3+ years clinical experience and 2+ years CM or discharge planning; Bachelor's preferred; comprehensive benefits discussed during application.

Qualifications

• Active and unrestricted CT license.
• Minimum 3+ years clinical practical experience (diabetes, CHF, CKD, post-acute care, hospice, palliative care, cardiac) with Medicare members.
• Minimum 2+ years CM, discharge planning and/or home health care coordination experience.
• Registered Nurse with active state license in good standing within the region where job duties are performed.

Responsibilities

• Acts as liaison with member/client/family, employer, provider(s), insurers and healthcare personnel.
• Coordinates case management activities for catastrophic cases and chronically ill members across the care continuum.
• Interacts with members/clients telephonically or in person, may visit homes or offices.
• Assesses medical and vocational status and develops care plans to optimize wellness and outcomes.
• Communicates with stakeholders and prepares documentation of case work.
• Collaborates with internal multidisciplinary team to maximize health outcomes.
• Outreach to treating physicians or specialists as appropriate.
• Provides educational and prevention information for best medical outcomes.
