CVS Health is seeking an RN Case Manager in Illinois to lead complex care coordination for dual eligible members. You will interact with members, providers, and families to assess needs and plan efficient care pathways.

The role emphasizes collaboration with the multidisciplinary team, documenting care, and promoting optimal wellness while supporting return-to-work goals. Remote work with up to 50% travel is expected.

Qualifications

• Active RN license in Illinois is required.
• 3+ years of clinical practical experience.
• Ability to work independently and remotely.
• Strong analytical and problem-solving skills.
• Excellent written and verbal communication.
• Proficiency with standard corporate software.

Responsibilities

• Liaise with members, families, employers and providers to coordinate care.
• Coordinate case management for catastrophic and chronically ill members.
• Meet with members telephonically or in person as needed.
• Assess medical/vocational status and develop care plans.
• Document case work and communicate with stakeholders.
• Collaborate with multidisciplinary teams to optimize outcomes.
• Outreach to treating physicians regarding course of care.
• Provide education and prevention information.
