Qualifications

• At least 5 years of inpatient case management experience; management background and/or CCM/ACM-RN certifications preferred.
• Strong clinical judgment, interprofessional collaboration, and leadership in complex care coordination.

Responsibilities

• Coordinate care for high-risk patients with complex medical and psychosocial needs.
• Collaborate with inpatient, outpatient, and community teams to develop discharge plans and prevent readmissions.
• Assess patients, use predictive analytics, lead interdisciplinary rounds, and engage post-acute providers and families.
