Job summary

Cleveland Clinic Lutheran Hospital is seeking a Registered Nurse Case Manager (PRN). You will work with interprofessional teams to facilitate effective transitional care coordination and patient advocacy. You will develop individualized care plans and lead in patient rounds and handoffs to ensure continuity of care.

Minimum qualifications include RN licensure, BLS certification, and three years of RN clinical experience. BSN preferred; CTM/ACM/CCM certification within one year is a plus.

Qualifications

• Graduate from an accredited school of professional nursing.
• Current state licensure as an RN.
• BLS Certification through AHA or Red Cross.
• Three years of RN clinical experience.

Responsibilities

• Collaborate with interprofessional teams across the continuum of care to coordinate transitions.
• Develop comprehensive, individualized transitional care plans with patient/catient partners.
• Lead and participate in patient rounds, huddles and committee meetings.
• Complete effective handoffs to ensure continuity of care.
