Job summary

Cleveland Clinic’s Main Campus is seeking a Hospice Coordinator to join a team dedicated to high‑quality patient care. You will process inpatient referrals, coordinate discharge plans, manage services, and support patients and families as they transition from hospital to home or a facility.

You will work 8:00am to 8:30pm, every 4th weekend and 2 holidays per year, with orientation at Main Campus for 6 weeks.

Qualifications

• Graduate from an accredited school of Professional Nursing.
• Current state licensure as a Registered Nurse (RN).
• One year of current clinic experience as an RN.
• One year of experience in an acute med/surg or critical care setting.
• BLS Certification through AHA or Red Cross.
• Ability to travel to hospitals and community referral sources.

Responsibilities

• Coordinate inpatient referrals into hospice and discharge planning.
• Educate potential customers about hospice care benefits and eligibility.
• Ensure patients and families understand the hospice benefit.
• Order medical supplies and DME; document in EMR during transitions.
• Perform admission assessments with physician and collaborate on care plans.
• Attend interdisciplinary team meetings and participate in care planning.
