Cleveland Clinic's Main Campus is seeking an RN Case Manager to collaborate with inter-professional teams and coordinate patient transitions across the continuum of care.
This full-time role supports comprehensive, individualized care planning, adherence to accreditation standards, and seamless handoffs to the next level of care. It follows a schedule of 8:00am to 4:30pm with 3 weekend shifts per quarter.

As an RN Case Manager, you will collaborate with inter-professional teams across the continuum of care to facilitate and ensure effective transitional care coordination. Essential to this role are the core values of patient advocacy, holistic assessment, care planning and the empowerment of patients and families to self-manage.
This is a full-time role. A caregiver in this position works 8:00am to 4:30pm. 3 weekend shifts per quarter.

Pay Range
Minimum Annual Salary: $69,600.00
Maximum Annual Salary: $106,132.50
