Job summary

Oak Street Health is seeking an RN, Case Manager to lead care management for complex patients in our Cleveland area clinics. This field-based, on-site role partners with PCPs to coordinate care, prevent avoidable admissions, and optimize resources.

Qualifications

• Current RN license in the assigned state is required; CCM certification preferred or obtainable within 12 months.
• 6–8 years of nursing experience with emphasis on care coordination and case management.
• Experience in transitional nursing, discharge planning, or home health is strongly preferred.

Responsibilities

• Lead care management activities in partnership with the PCP to coordinate care for complex patients.
• Drive care coordination to prevent hospital admissions and optimize resources.
• Manage transitions of care and ensure timely post-discharge assessments and reconciliation.
• Collaborate with Social Worker, Behavioral Health, and other care team members to align care plans.
• Document assessments and care plans in the electronic health record per standards.
• Lead in-person interdisciplinary care planning meetings with the care team.
• Educate patients and families and advocate for their needs.
