Job summary

CVS Health is seeking an RN Case Manager to telephonically and face-to-face assess, plan, implement, and coordinate case management for members. You will evaluate medical needs, facilitate wellness, and help transition to appropriate CVS Health programs and plans. Strong RN licensure and 3+ years of clinical practice are required.

This full-time role offers a comprehensive benefits package and a schedule Monday through Friday with possible evenings or weekends based on patient needs.

Qualifications

• Registered Nurse with active and unrestricted RN licensure in their state of residence
• 3+ years clinical practice experience required

Responsibilities

• Assess, plan, implement, and coordinate all case management activities with members
• Evaluate medical needs and facilitate wellness and care planning
• Review prior claims to assess impact on case management and eligibility
• Coordinate referrals to clinical resources and services as needed
• Collaborate with supervisor and multidisciplinary team to overcome barriers
