Job summary

Andwell Health Partners is seeking a Hospice Admission RN to provide skilled nursing services in patients’ homes in Lewiston, ME, under a physician’s plan of care. The role emphasizes comprehensive, person-centered care with a focus on comfort and quality of life.

Qualifications

• Current R.N. license in the State of Maine.
• ANA certification in Home Health or Community Health Nursing is encouraged.
• Current CPR card required.
• One year of recent clinical nursing experience is required.
• A reliable car with insurance.

Responsibilities

• Provide skilled nursing services to the patient in their own home under the direction of a physician’s plan of care.
• Develop plans of care with realistic/measurable outcomes in collaboration with the patient/family.
• Provide education and promote patient’s rights and responsibilities for informed decision-making in the plan of care.
• Assess and identify the patient’s holistic health care needs.
• Demonstrate competency in assessment and technical skills.
• Possess and utilize knowledge of pain control & symptom management.
