HCA Florida Sarasota Doctors Hospital seeks an experienced Case Manager Registered Nurse to coordinate care and support patients and families through every step of the care journey. You will partner with physicians, nurses, and leaders to ensure safe transitions and high-quality outcomes.

Qualifications

• Associate Degree in Nursing or Nursing Diploma required.
• Bachelor’s degree in nursing preferred.
• 2+ years in case management or 3+ years in clinical nursing required.
• Active RN license in the practicing state(s).

Responsibilities

• Guide patients and families through orientation, discharge criteria, and rights.
• Coordinate education for patients and caregivers for treatment planning.
• Develop and implement individualized treatment plans reflecting patient needs.
• Complete psychosocial assessments and document findings for the care team.
• Collaborate with Rehab Program Director and interdisciplinary team for patient-centered care.
• Promote HCA Healthcare values in every patient interaction.
