Job summary

HCA Florida JFK Hospital is seeking an RN Case Manager to coordinate patient care, develop individualized care plans, and facilitate smooth transitions. You will work closely with physicians, nurses, and department leaders to ensure safe, high-quality care across the continuum.

Qualifications

• RN must be licensed and eligible to practice in Florida.
• Experience in case management and discharge planning is preferred.
• Experience coordinating care across disciplines with physicians and nurses.

Responsibilities

• Coordinate services and develop care plans reflecting patient needs.
• Collaborate with Rehab Program Director and care team for seamless transitions.
• Educate patients and families about treatment plans and discharge criteria.
• Document psychosocial assessments and communicate needs to care team.
• Promote compassionate, respectful, and patient-centered care.
• Support patients and families through every step of the care journey.
