Job summary

HCA Florida St. Lucie Hospital is seeking a Registered Nurse Case Manager to support patients and families through their care journey, coordinating services and developing care plans aligned with individual needs.

You will work with physicians, nurses, and leaders to ensure safe transitions, clear communication, and consistent quality across the continuum of care, while upholding HCA’s values in every interaction.

Qualifications

• RN license in FL required.
• Associate degree required; bachelor’s degree preferred.

Responsibilities

• Guide patients and families through program orientation, rehabilitation philosophy, benefits, discharge criteria and patient rights.
• Coordinate education for patients, families, and caregivers to encourage participation in treatment planning and family conferences.
• Develop and implement individualized treatment plans reflecting patient strengths and recovery goals.
• Complete psychosocial assessments and discharge planning evaluations and document findings for the care team.
• Collaborate with Rehab Program Director, Facility Case Management Director, and interdisciplinary team for seamless, patient-centered care.
• Promote HCA Healthcare values of compassion, respect, and excellence in every interaction.
